If your clients need medical insurance as part of a travel insurance policy then TIS can issue policies, no age limit and with a medical screening which will cover pre-exisiting conditions. The only issue is where will they claim residency?
This option is cheaper than Private Medical plan for most pensioners.

PS When traveling from the UK you should get an EHIC card which replaced the E111.
